Registration is completed at the clinic

For new patients, we prefer that you visit Jan Family Clinic in person so our reception team can help you complete the registration and consent forms correctly.

  1. Visit the clinic during business hours or call ahead to confirm current physician availability.
  2. Bring your Ontario health card and current contact information.
  3. Complete the registration and consent forms at the clinic.
  4. Our staff will advise you about your first appointment and next steps.

For children and families

Our family physicians provide primary care for adults and children. Families may ask our reception team about registering household members together.

Once you are registered with the clinic, you may also use Medeo to book eligible appointments online.
