Examples include heavy menstrual bleeding, pregnancy, gastrointestinal blood loss, frequent blood donation, restrictive diets, and certain gastrointestinal conditions. The appropriate investigation depends on your age, symptoms, medical history, and individual risk factors.
How is iron deficiency diagnosed?
Your healthcare provider may order blood tests after reviewing your symptoms and history. Tests can include a complete blood count and measures of iron stores such as ferritin, with additional tests when clinically indicated.
Interpreting these results is not always as simple as looking at one number. Ferritin, for example, can be affected by inflammation and other medical conditions. Results need to be interpreted in the context of the whole clinical picture.
Iron deficiency without anemia
It is possible to have depleted iron stores before the hemoglobin becomes low. Some individuals may therefore have iron deficiency even though they have not developed anemia. Whether treatment is required, and what type of treatment is appropriate, should be determined by a healthcare professional.
How is iron deficiency treated?
Treatment depends on the severity of the deficiency, symptoms, underlying cause, tolerance of therapy, and other health considerations. For many people, oral iron is an appropriate first treatment. Your clinician may recommend a particular dose or schedule and may discuss ways to reduce gastrointestinal side effects.
IV iron may be considered in selected situations, such as when oral iron cannot be tolerated, has not produced an adequate response, absorption is impaired, or more rapid replacement is clinically appropriate. IV treatment is not automatically required simply because ferritin is low.
Do not forget the underlying cause
Replacing iron is only part of the management. If ongoing blood loss or another underlying problem is responsible, the deficiency may return unless that issue is addressed. Your clinician may recommend further assessment depending on your individual circumstances.
When should you speak with your doctor?
Consider medical assessment if fatigue, dizziness, reduced exercise tolerance, shortness of breath, heavy menstrual bleeding, or other concerning symptoms are persistent or affecting your day-to-day activities. If you have previously been told that your iron is low, follow the monitoring plan recommended by your healthcare provider.
Seek urgent medical attention for severe shortness of breath, fainting, significant chest pain, signs of major bleeding, or other severe or rapidly worsening symptoms.
